The Gatesworth, McKnight Place Extended Care, McKnight Place Assisted Living and Memory Care, and Parc Provence (collectively “The Gatesworth Communities”) are providing notice to potentially impacted individuals of a recent data privacy incident discovered on our network. The security of protected health information is very important to us, and we sincerely apologize for any inconvenience this may cause.

What Happened?

On or around February 5, 2025, we identified suspicious activity associated with certain systems in our network. As soon as we became aware of the activity, we took immediate steps to secure the environment and engaged outside computer experts to assist in our response and to investigate what occurred. The investigation determined that an unauthorized individual gained access to our systems from January 22, 2025 to January 26, 2025. The investigation also found that documents containing a limited amount of protected health information may have been copied from our network during the incident.

What We Are Doing:

In response to this incident, we have taken steps to minimize the risk of this happening in the future. We have changed passwords, implemented additional security measures on our network, and notified the FBI.

Not every resident’s information was accessed during this incident. We are currently investigating to determine who was impacted and what information was associated with those individuals. Letters will be mailed as soon as possible to impacted residents for whom The Gatesworth Communities has addresses, which contain more information about the incident, complimentary credit monitoring and identity protection services, and steps residents can take to protect themselves. We have no evidence that any of the information impacted by this incident has been misused, but we wanted to make you aware of this out of an abundance of caution.
